A Clinical Study of Gastric Cancer



Abstract
The study was a review and analysis of 868 cases of gastric cancer which were operated on the Department of Surgery, Hanyang University Hospital from May 1972 to May 1987 The results were as follows:
1. Peak age incidence of gastric cancer was 5th decade (30.3%) and male to female ratio was .1:1.
2. Chief complants of gastric cancer were epigastric pain or discomfort (81.3%), indigestion (46 1%), and weight loss (42.7%). In physical examination, epigastric palpable mass (24.1%), epii stric tenderness (21.1%) and anemia (10.9%) were discorved.
3. Before the admission, the duration of illness within 3 months was noted in 31.3% which was the most
common, however, longer than 2 years history was in 22.6%.
4. 55.9% of cancer patients were positive (above level of 2.5 ng/ml) in preoperative serum CEA evel. 5. At the examination of occult blood in stool, 72.5% of cancer patients were positive.
6. In the gastrofiberscope and radiologic examination (U.G.I. series) of gastric cancer, the positiv- fin
dings were found in 95.7% and 91.1% respectively.
7. The most common site of the gastric cancer was the lesser curvature of pylorus and antrum (45 4%). 8. 42.9% of gastric cancer were in the range of 6 to 10 cm in diameter.
9. The regional lymph node metastasis was seen in 64.9% of all cases. The distant metastasis or a irect
invasion was noted in the omentum, the pancreas, the mesocolon, the liver and the rectal sh if in
order of frequency.
10. The operative procedures performed were subtotal gastrectomy (78.7%), total gastrectomy (7 3%),
by-pass procedures (9.6%), exploation (4.4%). So resectability was 86.0%.
11. In pathologic findings, the most common type was adenocarcinoma (98.3%).
12. In postoperative complication,the pulmonary and wound complications were major complicatio and
the others were the intestianal obstruction, the intraabdominal abscess, the enterocutaneous fist, etc.
